CCARC’s Mission and Vision are applied every day through an array of adult day, employment, and residential services. We have a longstanding reputation for offering quality care, supporting personal growth, and offering continuity of services. Through Person-Centered Planning, CCARC works with the individual and their support team to determine the services that best suit their needs and abilities. Whether they require 24/7 extensive support, 1-1 care, or minimal/as needed assistance, CCARC provides the staff, resources, and settings to fulfill their needs and life goals.

Residental Services

RESIDENTIAL SERVICES

Our Residential Services support individuals in safe, happy, and comfortable community living environments. 

CCARC owns several homes that fall under Community Living Arrangements (CLAs). Once known as group homes, individuals live in accessible family-like environments that are staffed 24/7. Individuals live with roommates, or housemates who become their family. These homes stand out for being well-maintained, orderly, warm, and comfortable dwellings where people with disabilities share life together and are part of their local community. 

Continuous Residential Support (CRS) is another model of a 24/7 residential service. The distinction is the funding structure; however, the support and care the individual receives remain the same as in a CLA. In a CRS, the people living in the home hold the lease directly with the landlord. 

As we move towards more progressive community living options, CCARC is embarking on its first Cluster Living project. This opportunity allows people with I/DD to live with greater independence and autonomy in an integrated and safe environment. In this Developer/Private Provider partnership, a maximum of 25% of total apartments are designated for people with intellectual and development disabilities (I/DD). CCARC qualifies residents for these apartments and offers In-Home Support (IHS) services and an onsite staff presence for weekends and overnight. 

If your loved one still lives at home or lives independently, CCARC provides In-Home Support (IHS). This program provides limited staffing to assist the individual in their daily life. Examples of support range from transportation to a doctor’s appointment or bringing them to the grocery store to helping them pay their bills. 

 

No matter the chosen setting CCARC provides support based upon the needs of the individual. Direct Support Professionals (DSPs) work with residents to assist them in reaching their goals. Support is given in all aspects of daily living and community and social interaction. These experiences enhance personal growth and foster confidence when interacting with the community. Assistance is provided with learning, retaining, and improving daily living skills. Additional support may include:

 

  • Mobility or travel training

  • Addressing medical or behavioral needs

  • Enhancing their ability to access the community

  • Personal hygiene and care

 

Adaptive and assistive technology may be used to supplement support and independence within each of these settings. The CCARC clinical team addresses medical and behavioral needs.

CCARC provides Day Support Options (DSO) that focus on community inclusion and skill development. We help individuals actively participate in the community, socialize with their peers, and enjoy leisure activities. Participants benefit from life skills training, volunteer work, vocational training, and recreations/cultural events.

 

Opportunities to explore and interact within the community may include: 

 

  • Educational trips and museums

  • Exercise programs, nature, and health walks

  • Gardening and other outdoor activities

  • Arts and crafts, and music programs

  • Multi-sensory stimulation activities

  • Community volunteer work.

CCARC employs a team of nurses and behaviorists who use the Person-Centered approach to address the healthcare and behavioral needs of people participating in both our day and residential programs. 

 

CCARC’s behaviorists collaborate with psychiatrists, psychologists, and clinicians to create effective and innovative plans promoting optimal quality of life and encouraging maximum independence for individuals. Plans emphasize a proactive approach utilizing strategies and coping skills to build strengths and foster growth.

 

Healthcare needs are monitored round-the-clock by onsite nursing staff, as well as on-call nursing support. This team monitors the healthcare plans of group home residents, provides support and trains staff in the administration of medications and other healthcare-related duties, and consults on medical issues for CCARC participants.
